Cityblock Health was built to serve the communities that traditional healthcare overlooks. Founded in 2017 as a spinout from Alphabet’s Sidewalk Labs, the company delivers primary care, behavioral health, and social services to Medicaid and dually eligible (Medicare/Medicaid) populations — people who often face housing instability, food insecurity, and transportation barriers alongside their medical needs.

The model combines community-based care teams with a proprietary technology platform called Commons. The platform integrates medical, behavioral, and social data to create comprehensive member profiles. Care teams include physicians, nurses, community health workers, behavioral health specialists, and social workers who coordinate through the platform and meet members wherever they are — in homes, shelters, community centers, or virtual visits.

Cityblock operates in several markets including New York, Connecticut, Massachusetts, Washington D.C., Indiana, and North Carolina. It partners with Medicaid managed care plans, taking on full or shared financial risk for member populations. The company raised over $900 million in total funding, with investors including General Catalyst, a]6z, and Alphabet. The team has grown to over 1,500 employees. The core bet is that investing in social determinants — helping members get stable housing, food access, and behavioral health support — dramatically reduces emergency visits and hospitalizations, lowering total cost of care while improving quality of life.
